Pipeline Safety Tip of the Month -8-1-25- Top Distribution UNSATS

I used to belong to the NASPR-PHMSA Distribution Team.  In 2022 that team presented the top unsatisfactory inspection questions for gas distribution operators nationwide.  That information is represented below.  My own experience parallels these findings with inadequate procedures and missing records my top findings.  How can you use this information to minimize your unsatisfactory inspection results in the future?
